What is required to do a 4H Field Trip?
A List of members attending along with signed medical release forms, and field trip permission slip is required to be collected.
Only registered on-line volunteers are allowed to driver other 4-h'rs.
A parent who is not a volunteer can only drive their own child in the car.

Parent Volunteers
Our club is always looking for parent volunteers. To become a volunteer the three steps need to be completed with the 4-h Office.
1) Obtain fingerprinting form and get fingerprinted
2) Complete 4-H On-line Registration. Registration starts Sept 1st and ends Oct 15th.
3) Complete the Leader Training Form

Registration in done on-line and payment needs to be brought to the Fair Lane 4-H with your waiver forms signed.
If you are an adult volunteer, please turn your Adult Medical Release fForm into your club leader. All 4-h members are required to turn in the Youth Medical Relase Forms to their club leader and a second copy to their Project leader.
